Sunrise Eye Care Appointment policies
Our goal at Sunrise Eye Care is to provide quality care to our communities. There has been an increased demand for eye care in our area, therefore in order to best accommodate our current established patient base, we are currently unable to take on any new patients to our practice at this time.
APPOINTMENTS:
Annual eye exams and follow up visits are by appointment only. Emergency visits will be scheduled at the discretion of the doctor on site. We ask that all patients arrive 10 minutes before their scheduled appointment time.
Please bring the following with you to your appointment:
*Payment for services will be due upfront if insurance cards are not presented at the time of service. We strongly encourage you to familiarize yourself with the specific details and copays associated with your insurance plan. Choosing to forgo your designated appointment time rather than provide required payment as specified by your specific insurance plan will be considered as a no-show.
CANCELLATIONS & NO-SHOWS:
We understand that there may be times where an appointment will be missed due to an emergency or sickness. However, when you do not call to cancel your appointment in advance you may be preventing another patient from receiving care. If it is necessary to cancel your appointment we kindly request that you cancel at least 24 hours in advance. Appointments are in high demand and your early cancellation will give another person the opportunity to be seen.
A "No-Show" is someone who missed an appointment without advance notice that they intend to cancel their appointment. No shows will be documented, and after two no-shows we reserve the right to require a deposit up front in order to schedule you for any future visits. More than two no-shows within the year may be cause for dismissal from the practice.
LATE APPOINTMENTS:
We understand that delays can happen, however if you arrive more than 10 minutes past your scheduled appointment time, we may need to reschedule your appointment in order to keep on time for our other scheduled appointments.
