Sunrise Eye Care Appointment policies
Our goal at Sunrise Eye Care is to provide quality care to our communities. There has been an increased demand for eye care in our area, so in order to best accommodate our established patient base, we are unable to take on any new patients to our practice at this time. We will always see children under 18 years of age who are new to our practice, and will do our best to accommodate eye emergencies for established and new patients to the best of our ability.
APPOINTMENTS:
Annual eye exams and follow up visits are by appointment only. We ask that all patients arrive 10 minutes before their scheduled appointment time.
Please bring the following with you to your appointment:
*Insurance cards must be presented at the time of service. We strongly encourage you to familiarize yourself with the specific details and copays associated with your insurance plan. Choosing to forgo your designated appointment time rather than provide required payment as specified by your specific insurance plan will be considered as a no-show.

Missed appointments delay care for others and make it harder for us to meet the growing demand for optometry services in our area. We appreciate your understanding and cooperation in helping us to provide care for as many patients as possible.

- A current list of medications

1. CANCELLATION & RESCHEUDULING:
- Please provide at least 24 hours notice if you need to cancel or reschedule your appointment. This will allow us to offer your appointment time to another patient in need
- A "no-show" is when a patient misses an appointment without notifying us in advance
- First Occurrence: we understand emergencies happen, we will remind you of our policy and help you reschedule
- Second Occurrence: For all future appointments we will require you to physically confirm your appointment with us (by phone, in person, or in writing) at least 24 hours in advance. If we do not receive confirmation, we will offer your appointment time to another patient.
- Two or more no-shows in a calendar year will impact your ability to reschedule any future appointments with our practice and may be grounds for dismissal.
- We understand that delays can happen, however if you arrive more than 10 minutes past your scheduled appointment time, we may need to reschedule your appointment in order to keep on time for our other scheduled appointments.
